please have a look at
aka the bio_clone bug, introduced in mainline kernel 2.6.11-rcX and
fixed in mainline 220.127.116.11 (so it is probably present in debian
18.104.22.168, which you happen to use).
there is also a thread about this in this mailing lists archives,
about August 5.
I'm not saying it is this bug, I just suggest it might be.
